Does Kratom Show Up on a Drug Test? 5-Panel, 10-Panel, DOT and Urine Tests
The answer depends on the exact analytes ordered—not simply the number printed on the drug-test panel.

People rarely search this question casually. A pre-employment screen may be approaching. A driver may be worried about DOT rules. Someone in a treatment or court program may not know what its expanded panel includes. The anxiety is understandable, but a generic panel name cannot predict an individual result.
Drug tests are built around selected targets and cutoff concentrations. They do not automatically identify every botanical, prescription medicine or psychoactive compound in a specimen. At the same time, “not included on a routine panel” does not mean “impossible to detect.” Dedicated kratom testing is commercially available.
1. Does kratom show up on a standard drug test?
Most routine workplace panels do not specifically target kratom. Kratom comes from Mitragyna speciosa. Its best-known alkaloid is mitragynine, while 7-hydroxymitragynine is usually shortened to 7-OH. These compounds are not the same as the traditional opiates targeted by many routine immunoassays.
A standard test only looks for the substances included in that test’s design. A laboratory can nevertheless test for kratom when mitragynine or another relevant marker is ordered. That is why the correct answer has two parts: routine tests generally omit kratom, while targeted tests can identify it.
Kratom’s availability or legal status does not decide whether an employer, court, clinician or treatment program can include it in a testing policy. Product legality and testing policy are separate questions.
2. Does kratom show up on a 5-panel drug test?
A routine 5-panel test generally does not include mitragynine. Traditional panels commonly cover marijuana metabolites, cocaine, amphetamines, specified opioids or opiates, and PCP. A program can still order an additional assay, so “5-panel” should not be interpreted as proof that no other testing will occur.
The safest practical question is simple: Does the complete laboratory order include kratom, mitragynine, 7-hydroxymitragynine or an expanded toxicology panel?
3. Does kratom show up on a 10-panel drug test?
A 10-panel label alone cannot answer the question. Many commonly sold 10-panel tests do not include kratom, but non-DOT panels are not one universally fixed analyte list. Different vendors and programs can package different targets under similar panel names.
This is where several online explanations become misleading. One article may say a 10-panel always detects kratom. Another may say it never does. Both statements are too broad. If the order specifically includes mitragynine or a kratom assay, detection is possible. If it does not, the extra panel positions may be assigned to unrelated substances.
4. Does the DOT test for kratom?
The current DOT-mandated panel does not list kratom or mitragynine. The U.S. Department of Transportation identifies marijuana metabolites, cocaine, amphetamines, specified opioids and PCP within its regulated panel.
That does not necessarily end the policy question. A DOT-covered employer may also maintain a separate non-DOT testing or fitness-for-duty policy. Drivers and other safety-sensitive workers should review both the federal requirements and their employer’s written rules rather than assuming the DOT panel is the only standard that applies.
5. What does kratom show up as on a drug test?
When testing is designed specifically for kratom exposure, the result may identify mitragynine, a metabolite or another kratom-related marker selected by the laboratory. A result should not automatically be expected to appear under the broad word “opioids.”
Product names can create additional confusion. Plain kratom leaf products and concentrated products marketed around 7-OH should not be treated as analytically identical. 6. How do laboratories test urine for mitragynine?
Testing can involve two different stages:
- Presumptive screening: An immunoassay is designed to indicate whether a selected target may be present above a defined threshold. Screening methods can be useful, but they are not perfectly specific.
- Definitive confirmation: A laboratory may use chromatography with mass spectrometry to identify a particular compound more specifically. Confirmation is especially important when an initial result is unexpected or disputed.
This is not merely theoretical. Labcorp lists a kratom/mitragynine urine test with screening and confirmation. Quest Diagnostics also lists mitragynine urine monitoring with confirmation. These examples prove that targeted testing exists; they do not prove that a particular employer or program orders it.
7. Does a kratom test detect 7-OH products too?
Not every kratom test directly targets 7-hydroxymitragynine. Some available screening methods target mitragynine instead. In June 2026, Labcorp reported that its kratom immunoassay does not directly detect 7-OH, while many products marketed around 7-OH also contain mitragynine and may therefore still produce a positive mitragynine screen.
This distinction matters because “kratom test,” “mitragynine test” and “7-OH test” should not be assumed to mean exactly the same assay. Ask which alkaloid or metabolite the laboratory targets and whether confirmation tests for the same marker.
8. How long can kratom be detected in urine?
No universal number can predict an individual urine result. Commercial testing sources commonly describe a multi-day detection period, often around five days, while some consumer-facing sources report longer estimates. Those numbers depend on the assay, cutoff, specimen timing and the population being discussed.
Other variables can include frequency and amount of exposure, product composition, metabolism, health factors and whether the product contains ingredients not accurately represented on its label. A calculated half-life is not the same as a guaranteed testing window.

9. Can kratom cause a false positive for methadone or opioids?
Kratom should not be described as routinely causing an “opioid positive.” Biological activity at an opioid receptor is not the same as analytical cross-reactivity in a drug test. Different immunoassays use different antibodies and target definitions.
A published pathology study found that a kratom metabolite could interfere with one Thermo Scientific CEDIA methadone-metabolite, or EDDP, immunoassay. That is an assay-specific presumptive-screen finding. It does not establish that kratom routinely creates positives across all methadone, opiate or opioid tests.
If an initial screen is unexpected, ask whether the sample received definitive mass-spectrometry confirmation and whether a medical review process applies. Confirmation can distinguish compounds more specifically than a presumptive class screen.
10. Do employers, probation programs or courts test for kratom?
Some may. Routine employment panels often omit kratom because adding a dedicated assay increases complexity and cost. However, an employer, treatment program, pain-management practice, court or probation program may request expanded testing when its policy allows it.
Do not assume that legal availability means a program must allow use. Do not assume that an herbal product is invisible to testing. The written policy, court order, laboratory requisition and responsible professional are the controlling sources for a high-stakes situation.

- Which specimen and testing method will be used?
- Does an initial non-negative screen receive definitive confirmation?
- Is there a medical review officer or documented appeal process?
- Does the policy prohibit use even if the routine panel does not test for it?
Do not attempt to change a specimen or rely on internet “detox” methods. Such tactics are unreliable, may violate policy or law, and can create a separate invalid- or adulterated-specimen issue.
12. Florida and Miami considerations
Florida regulates aspects of kratom sales, and local restrictions can differ. Rules affecting concentrated 7-OH products may also differ from rules applied to conventional kratom leaf products. Those legal distinctions do not decide what an employer, court, treatment program or healthcare provider may include in a testing policy.
